What are you best known for among patients?

My caring demeanor, technical accuracy and natural-looking results. I’m an artist and perfectionist when it comes to beauty and my patients, so I’m very adept with aesthetic injectables and lasers. What anti-aging advice do you like to pass along to patients?

Take good care of your body; it’s the vessel that holds your soul. I also tell patients to replenish, rejuvenate and exercise. In order to reverse the signs of aging, I always advise patients to start early, as doing so helps to avoid more drastic procedures.

What is stem cell therapy and how is it used to regenerate the face?

In our practice, we use pluripotent stem cells, extracted from ovine (sheep) placenta and umbilical cord tissue.  It is applied to the face and neck in the form of a serum. It is a completely natural and organic product, with no chemicals or synthetic additives. This clinically proven treatment allows your face to grow new skin cells, replacing ones that have been compromised through environmental stress, sun damage, or age. Is there a non-invasive alternative to liposuction for fat removal? How long do the results last? Do you need multiple treatments?

 The only one that I know that is worth trying is the Zeltiq method of Cryolipolysis.  This system requires at least two sessions per area to achieve the best results.

What is a Nefertiti Neck Lift?

The Nefertiti neck lift is a new technique achieved by placing multiple injections of Botox along the lower jaw line as well as down the side of the neck along the platysma muscle. This improves the definition of the jaw line and rebalances the underlying muscles to provide ‘lifting’ in the neck and jaw area. You will notice improved tightening and overall reduction of drooping neck skin and often reduced wrinkles on the neck in as few as three to five days.

What is the best laser for hair removal?

There are numerous lasers for hair removal; however, one should know that in order to efficaciously and safely remove hair via laser, one should choose a wavelength that has the most affinity for melanin and also respect the skin tone of the patient.  The 755 nm wavelength is by far the most effective wavelength to destroy hair in all shades of light skin, especially in people with finer and lighter hair.  The 1064 nm wavelength is the best and safest to treat very dark-skinned individuals.  In our practice, we use the Elite and the Elite Multiplex laser (Cynosure Inc.) that uses both 755nm and 1064 nm wavelength.  This laser has won “the best laser for hair removal of its kind” for 7 years in a row.

STRETCH MARK TREATMENTS

What is the best treatment for stretch marks? Scars?

In my practice, they are best improved by the Affirm Multiplex technology.  Using the Fractional C02 also improves them; however, it tends to cause prolonged redness and hyperpigmentation, which causes concern in patients.  Stretch marks are very difficult to treat and require multiple sessions, but recently acquired stretch marks (such as after pregnancy) will respond better and faster than old stretch marks.

ACNE TREATMENTS

How do you treat acne scars? Pock marks?

Acne scars are easily improved with the fractional C02 technology or non-ablative 1440 nm Affirm Multiplex for those that do not want to have too much down time.  If a patient has ice pick scars, I may use a drop of 50% TCA chemical peel into the pit, before I do the lasers.  This will help seal and flatten the ice pick scar, so the laser will later further improve them.

CELLULITE TREATMENT

How do you get rid of cellulite? How long do the results last? Do you need multiple treatments?

Until now, with the introduction of the Cellulaze laser, most treatments for Cellulite have been non-invasive with modest to great results, but all have been short lasting.  The Cellulaze laser was just recently approved by the FDA, and has shown in Canada and Europe to be a more permanent solution to treat this multi-dimensional disease.  It works by lysing the fat cells and eliminating the bulging fat that cause the peaks on the topography of the skin, destroying the cords of tissue that are pulling and puckering the skin down, and by increasing the amount of collagen in the skin improving the elasticity and texture.

How do I get rid of brown spots caused by sun damage?

There are different wavelengths that can target melanin.  In our practice, the most used wavelength is the 755 nm Alexandrite laser, because it has the most affinity for brown pigment.  Sometimes the brown pigment may be a little too light (tan spots), which do not contain enough melanin for the Alexandrite laser to see it.  Those tan spots can be treated with other modalities that tend to peel off the pigment, instead of breaking it down.  For those tan spots, an Erbium-Yag laser can be used, as well as the fractional C02 laser, chemical peels or bleaching agents.

What is the best laser for total facial rejuvenation? How many treatments do I need? How long do results last?

The Fractional Co2 technology is by far the best technology to mechanically rejuvenate the skin.  Treatment needed depends on the extent of the photo damage and the age of the patient.  However, with the fractional C02, impressive rejuvenation results are achieved after only one treatment.  In my practice, I use the Smart-Skin Co2 with multi-pulse technology, which allows me to go deeper into the skin with only one pass, giving superior results with less discomfort to the patient.  It only requires one pass instead of the usual two passes required by other technologies.

What is the best treatment for facial rejuvenation with the least amount of down time?

The non-ablative lasers and RF (radio frequency) are the best to rejuvenate the skin with minimal to no downtime.  In my practice, we use the Affirm Multiplex laser with the capability to do both collagen remodeling and tissue tightening.  In addition, we also use the Pelleve and Accent Xl technology to increase tissue tightening for a more youthful facial contour.

Can you rejuvenate skin on the rest of the body with lasers, or just the face?

Yes, you can.  Skin is skin and it will respond to any of these technologies.  People tend to have more photo damage and aging on the face, neck, chest and hands.  I have many requests for skin rejuvenation of the knees and elbows as well.

What is the best treatment for deep wrinkles? Fine lines?

For deep wrinkles, the fractional C02 is your best bet.  For fine lines, the 1440 nm wavelength from the Affirm Multiplex is a good choice.  However, most non-ablative photo-facials will have a positive effect on fine lines.  We are patiently awaiting the launch of the first approved “true line filler,” called Belotero.  This filler will instantly diminish wrinkles.

What is the best laser to treat redness (vascular conditions)?

The 585-595 nm Pulse dye laser and the 1064 nm Nd:Yag lasers are the best wavelengths to treat vascular lesions.  In our practice, we use the Cynergy laser with multiplex technology. This laser uses both wavelengths sequentially (585-595 nm and the 1064 nm), one right after the other.  This allows us to use less energy from each wavelength, making this treatment more efficacious and safer to the patient.  In my opinion, this is the best vascular laser that exists in the industry.  We use this to treat redness on the face, vascular lesions such as hemangiomas and port wine stains, as well as to treat Rosacea, Psoriasis and acne.

What is the Affirm laser and how is it different from other lasers?

The Affirm laser (Cynosure Inc.) is a multiplex laser technology that uses 1440 nm and 1320 nm wavelengths for total non-ablative skin rejuvenation. The 1440 nm wavelength is a versatile wavelength that it is used to remodel old, sun-damaged collagen and to promote and increase the amount of new collagen in the dermis.  This wavelength is also helpful in treating other conditions such as melasma, stretch marks, acne scars, keloids, and hypertrophic scars.  The 1320 nm wavelength is for tissue tightening and it is used in conjunction with the 1440 nm wavelength.  This creates a complete treatment for facial rejuvenation that improves wrinkles and skin texture as well as tightens saggy skin.

How do LED lights treat the skin?

Light Emitting Diodes (LEDs) are used for the treatment of a wide range of dermatological conditions including, acne, photodamage, non-melanoma skin cancers, skin rejuvenation, vitiligo and wound healing post-elective surgery. LED light is delivered in pure, optimized, continuous narrowband light via a matrix of LEDs carefully positioned to deliver light gently, safely and effectively to the treatment area. 

At Shino Bay Cosmetic Dermatology & Laser Institute, we use all of the best leading edge technologies such as the Omnilux RED and BLUE LED technologies. We often utilize the RED LED to minimize and prevent bruising, swelling or redness that may be associated with post laser or aesthetic injectable procedures. Quick 10 to 20 minute, completely soothing LED treatments can easily diminish or prevent what we refer to as "Lasacea" (redness post laser) as well as, induce healing and often improve speed of healing by 50 percent or more. The red light also has rejuvenating effects on the skin by promoting new collagen formation and increasing the micro circulation of the skin.

The blue light helps diminish acne by killing bacteria.  The blue light causes the bacteria’s by-product (waste) to become toxic to them.  In our practice, we use the blue light in combination with the red light.  The blue light kills the bacteria and the red light reduces inflammation and promotes healing.  In addition, the blue light can be used with Levulan (a photosynsetizer), for photodynamic therapy that we reserve for severe cases of acne vulgaris.

How does Radiofrequency treat the skin?

Radio frequency technology delivers heat to deep layers of the skin where the skin and fat layer meet. This "controlled damage" spurs the contraction of these deep tissues for an overall smoothing effect.

The heat energy also stimulates the production of new collagen over time, resulting in firmer, tighter, smoother skin. The practitioner adjusts the strength of the delivered energy based on feedback from the patient throughout the session.

What is the best laser for treating dark skin?

For laser hair removal, the 1064 nm wavelength is the best one to treat dark skin tones.  The 1440 nm is safe to use on dark skin for rejuvenation, but a test spot is recommended.   

What is the difference between Fractional Ablative, Full Ablative and Non-Ablative Fractional laser technologies?

Full ablative technology has been around since the early eighties. Ablative simply means that the top layer of the skin is burned off to reveal smoother, younger-looking skin. The heat generated from the laser stimulated new collagen formation giving a smoother appearance to the skin.  This technology gave great rejuvenating results, but the down time was very long (1 to 2 months and sometimes even longer), and adverse reactions were not uncommon.  The most common adverse reaction was discoloration of the treated area (either too white, too dark or too red) and scarring.  This technology was reserved for light skin types with severe photo damage.

Fractional ablative is a technology where only micro areas of the skin are harmed by the laser and other areas are left unharmed.  The advantage of this technology is that wound healing begins rapidly, so the down time is greatly reduced.  For example, with the Smartskin C02 in my practice, the down time is approximately 5 to 7 days instead of the 1 to 2 months for the full ablative technologies.

Non-ablative technologies like the Fraxel or Affirm Multiplex works by creating micro injuries to the skin without burning off the top layer of skin.  This technology basically helps remodel old sun-damaged collagen and increases the formation of new collagen fibers, giving a more refreshed look to the skin and reducing the appearance of wrinkles.  This technology can be used as a preventative treatment as well as a corrective treatment.

What is the best treatment to tighten the skin?

It all depends.  If the skin itself is feeling loose and crepey, then using a non-ablative or ablative  laser will help to solve this problem; however, if the skin feels lose or basically hanging, then using a device that goes down to the subcutaneous tissue will be more appropriate (like the radio frequency technologies) .  In our practice, we tend to use a combination of these technologies.  The Affirm Multiplex has a wavelength that helps generate new collagen (1440 nm, Nd:Yag) and another one that tightens the skin (1320 nm Nd:Yag).

What is a non-surgical eye lift? How is it performed?

A non-surgical eye lift uses laser and RF (radio frequency) technology to create new collagen and tighten the skin around the eyes.  In our practice, we have different modalities to achieve this depending on the severity of the case.  It can be done with the fractional C02, the Affirm multiplex and RF technology such as Pelleve and Accent XL.  All these modalities can be used alone or in combination.  The best results are achieved by doing packages where laser and RF technology are used synergistically.
