Natural-looking Nose

Regularly referred patients by other physicians for rhinoplasty, a procedure with a relatively high revision rate, Dr. Hecht stresses the importance of going to a plastic surgeon with extensive experience in rhinoplasty. “A poorly performed rhinoplasty can have negative functional outcomes,” Dr. Hecht says. “Also, the nose is not as forgiving as other parts of the body. Its rigid cartilage and bone don’t mold over time so imperfections can actually become more prominent versus soft tissue and scarring, which have the capability to improve over time.” Additionally, many patients don’t realize how big a role their skin plays in the outcome of a rhinoplasty. While thin skin typically shows the changes more dramatically, thick skin is better at hiding imperfections. “Ideally you want medium thickness, but you always have to work within the parameters that each individual’s skin will allow,” Dr. Hecht says.

About my practice:

From what vantage point do you approach patient care?

I perform time-tested surgical and non-surgical procedures that will offer the best possible result for each patient, always taking personal needs, preferences and considerations into account.

What is your goal for each patient you treat?
I strive to create an un-operated look, tailoring treatment to each patient's anatomy and goals. My approach involves preserving or enhancing function in addition to maximizing aesthetic outcomes.
How would you describe your operating room style?

It's controlled and meticulous.

What is a consultation with you like?

I personally evaluate each patient and allow ample time during consultations for a comprehensive discussion of all treatment options, using photography and computer imaging. I am a careful listener, and work very hard to understand each patient's goals.

What techniques do you use to rejuvenate the face?

I tend to customize my facelifts and techniques depending on my patients' needs. Once we see extra skin and looseness in the neck, the only reliable way to tighten it is with a facelift. In the mid-face, I can use fillers like Radiesse® and Juvéderm® to restore volume, and, finally, i can treat the forehead with Botox, or, when necessary, a browlift.

I perform pro bono work for face to face, a program that helps victims of domestic violence and those afflicted by deformities and physical limitations. I was also selected as one of 'The Best Doctors in America' for 2011-2012 and have also been recognized multiple times by my peers as a Top Doctor in Phoenix Magazine in 2007, 2009 and 2010.

Why did you become a facial plastic surgeon?

I am a perfectionist, so I picked a field where that attribute is appreciated.

How do you get rid of sagging skin in the neck?

Surgically with a necklift if there is significant skin laxity or hanging skin. If skin laxity is mild, people can try lasers, but I’m not really in favor of that.

What is the best non-invasive treatment for overall skin rejuvenation

We like to put patients on medical-grade prescription skincare products with regular treatments of light peels and microdermabrasion by the esthetician. We can also treat them with a photofacial-type laser.

What is the difference between getting microdermabrasion, a chemical peel or a laser treatment? Which one should I use?

Deeper peels are done by the physician; but if they need more of a superficial treatment than it is done by our esthetician, Rihanna Lankau. Lankau says, “Microdermabrasion is a mechanical treatment that removes dead skin cells. It’s like a pick-me-up for the skin. There are a variety of peels. I do medium-depth peels, but ‘super’ peels are done with Dr. Hecht.  Chemical peels are good for melasma because lasers can cause darkening of the melasma.  Peels also help produce collagen and elastin. Lasers are great for sun-damaged skin. They also help produce collagen in the skin.” Lasers are also really good for treating redness caused by vessels such as rosacea, spider veins, angiomas or other red spots on the face.

Do I need a cast after a nose job? What about packing?

I use a really small plastic splint on the bridge of the nose with some tape.  The patient wears this usually for about a week (5-7 days).  Sometimes we use a small amount of packing overnight. 

How do you create a natural looking nose that will fit the rest of my face?

First of all try, we try not to over-resect and try to preserve the structure (the cartilage and bone) for support; so over long-term the nose doesn’t lose support and become too small. A lot of times we use cartilage grafts to reshape the nose and to maintain structure and support. We try to balance it with other facial features like with the chin and the size of the face. 

How long until I can go out in public after a nose job

Patients usually wait until the cast comes off, which is about a week. They can go out before that with the cast on, but it’s up to them.

Can blepharoplasty fix crepey skin in the eye area?

Typically if there is crepey skin we use a peel or a laser to tighten the skin in addition to removing fat, if it’s present.

Can a laser be used to eliminate fat under the eyes or is it best to use an incision?

I like to use an incision.  We use the laser for skin tightening if we need to.

How do you know if you need eyelid surgery or a browlift to fix sagging upper eyelids?

We assess that by the location of the brow itself, depending on if the patient is male or female, and determine if the brow is at or above or below the bony rim.  A lot of times we use Botox to get a browlift.  Most patients benefit form blepharoplasty (eyelid surgery) and then that can be augmented by either Botox or a browlift procedure if there is significant brow ptosis (drooping).  We prefer the endoscopic browlift technique on most patients, but we do use the other techniques, like an open browlift, as well.

FACELIFT

Is anyone ever too “far gone” for a facelift?

If they are medically fit than anyone can benefit.  A patient who does not have as much laxity might have a better result, but others would still have improvement.

What is the best alternative to a facelift?

For sagging skin in the neck, than a facelift is the only way of correcting that. If the patient is not interested in surgery, we can offer a variety of fillers to give a liquid-type facelift effect; especially in the mid-face region and along the jawline.  However, once there is hanging skin in the neck, there is no reliable way other than surgery of correcting that.

How long does a facelift generally last?

In most people between 5 to 10 years. Patients will always continue to age; but after a facelift, they will always look younger than they did to begin with.

How do you get rid of jowls?

Surgically as part of the facelift we tighten the skin along the jawline. We sometimes do some liposuction for the jowls as well. A chin implant will also fill in the hollow in front of them, smoothing out the jawline. Non-surgical options would be fillers, to help camouflage and blend in the jawline.

How do you get rid of a double chin?

It depends on the age of the patient.  If they are younger and have a little excess fat, we can do liposuction. If it’s an older patient and they have extra fat, skin and muscle looseness, we can address the problem with liposuction and facelift or neck surgery.  We can also use a chin implant to strengthen the jawline.

How do you get a more prominent chin?

Most commonly we use a chin implant; occasionally, we use fillers.

What is the best fix for deep smile lines?

We get really good results with hyaluronic acid fillers like Juvederm.

How do you get rid of a double chin?

How do you get rid of under-eye bags? Hollows?

Depending on the patient, we either remove fat through the inside of the eyelid or we make an incision on the eyelid. Sometimes we will also use a camouflage technique with fillers to help blend the eye with the cheek. For hollows, we usually use a hyaluronic acid filler like Juvederm.

How do you treat wrinkles around the eyes?

If they are in the crow’s feet region, we use Botox. For under the eyes, we use filler and resurfacing with either a peel or laser. We also use topical eye creams

Where do you use Botox?

The most common areas are the vertical lines between the eyebrows (glabellar lines) and on crow’s feet. When both areas are done together, they give a browlift-like effect, minimizing forehead injections.  It is also used for the platysma bands in the neck and to treat forehead lines, but you have to be conservative. It is also used very conservatively for vertical lip lines. 

If you already have forehead lines, will still Botox work?

If you already have lines, we first try conservative Botox treatments. If the lines are persistent, we can do some filler. If there is significant brow ptosis (drooping), than that would have to be corrected; or they might be raising their eyebrows subconsciously to try to compensate for sagging eyelids and would need blepharoplasty (eyelid surgery).
