What makes your practice distinct?
The privacy and intimacy of where I practice is a great comfort to patients. The freestanding 9,000-square-foot facility is complete with a secluded backdoor exit that allows patients to arrive and leave discreetly. Additionally, patients appreciate being in an environment dedicated solely to aesthetic surgery.
How do you make patients feel comfortable in your care?
I subscribe to a philosophy that treats each and every patient with dignity, imparting star-treatment to everyone who walks through the door.
How would you describe your approach to plastic surgery?
I believe that less is more. To patients who are worried about looking fake, I always say, 'you only notice bad surgery.' Good work should go undetected. That is what I strive for: natural-looking results that make someone look better but not like they've had surgery.
How would you describe your operating room style?

It's relaxed and focused. There are never wasted movements.

What is the treatment for men with an overdevelopment of tissue in the breast area?
This is called gynecomastia, and it can be one of three forms. It can be just fatty, glandular (breast gland), or a combination of the two. If it's fatty, the area is treated with liposuction. If it's glandular, it can't be removed with liposuction, and has to be excised directly. If it's both, we use a combined approach. If it's mostly fat we can suck most of it out, and then make an incision and take out the breast gland. I can tell by feeling the breast which on it is. It usually depends on the patient's age. In younger patients, it's usually glandular and we wait four years for it to revert on its own, and then we would operate on it. In middle-aged men it's usually fatty tissue. It also depends on the patient's history of using thing like steroids or marijuana.
How do you correct separated abdominal muscles?
It usually occurs after multiple pregnancies where sometimes the muscles stretch apart. You can do sit-ups all day long and your abs just don't return to where they once were. Called diastasis recti, this is something commonly done during a tummy tuck where the muscles need to be sewn back to the midline. We make a low incision from pubic area to the belly button. (If the incision is up to the sternum it's considered a full abdominoplasty.) Sometimes this procedure can be done without removing any skin.

LIPOSUCTION

What can patients expect after liposuction? Are the results immediate?

Results are immediate, but the final result takes several weeks or months to see. The patient will always look better than they did before the surgery, but it takes three to four months for swelling to go away. I have patients wear compression garment for three weeks afterward, but it's up to the surgeon. Some have patients wear them longer and some less.

LIP ENHANCEMENT

What is a lip lift?

It's any procedure that will shorten the distance from the base of the nose (or bullhorn) to the superior border of the lip, which enhances the look of the lip. An incision is made under the base of the nose. You can also make the scar along the border of the lip, but it's risker as scar. There's also the V-Y advancement surgery where the surgeon makes V-shaped cuts on the inside of the lips, sliding the V-shaped wedges forward, and then stitching them back up, creating a Y shape. The result is a fuller lip. From personal experience, the best procedure is where the incision is at base of the nose, or bullhorn.

Who is a good candidate for a lip lift?

Women who have noticed with aging that their upper lip has drooped. Usually works best on people with fairer skin, who are not prone to hyperpigmentation or keloiding of scars. Those women see drooping in usually in their 40s and 50s; but also women in their 20s and 30s who feel they have too long of an upper lip call also have a lip lift. It's all about proportions and what ratios are aesthetically pleasing to the eye.

Why would you have a lip lift instead of using facial fillers?

They are doing two different things. A filler plumps the lip up. It shouldn't be applied along the lip border which looks unnatural, but inserted right into the red part of the lip. A lift doesn't make the lips larger; it just pulls the upper lip up so the distance between the lip and nose is shortened. In doing so, it changes the shape of the lip. Not everybody needs a lift procedure. Some people just need volume.

How do you fix 'turkey wattle or weakened neck muscles?

It has to do with the degree of aging that you see. If you see the separation of platysma bands (neck muscles), is minimal and the skin is still elastic, you can do platysma band surgery where the incision is made under the chin. The skin is separated from the muscle and sewn like a corset to the midline. If there is skin laxity along with weakened muscles, than you need to address the skin either by doing a neck lift or facelift. Those incisions are made from the base of earlobe, back behind the ear; and then the skin and muscle are pulled back and tightened. That's what contours the neck. In some rare cases, there are men who choose not to have this procedure, and want just the skin excised, which can be done as well.

BREAST LIFT

How do you correct asymmetrical breasts, where one is larger than the other?

It depends on which breast the patient likes more. If they like the smaller one, then we do a breast reduction on the other side. If it they like the bigger one better, we can put an implant in the other side. But, it's usually more complicated. Sometimes we need to use two different sizes or shapes of implants, or we just add more saline. Sometimes the problem is with the nipple position. We have to tailor it to what the patient needs.

How is a breast lift performed?

There are degrees of the procedure. The lift itself uses a flap that supplies blood and nerves to the nipple areola complex. Then the breast mound is sewn together, tightened and reshaped to give a higher breast. Traditionally, this was thought of as a 'skin operation, but over time, the breasts would get droopy again. When we reshape the whole breast gland instead, then the skin doesn't stretch out again. This gives patients longer results.

BREAST REDUCTION

What are short scar and inverted "t" breast reduction surgeries and who is a candidate?

It depends on the surgeon and the choices. Short scar incisions can be used on breast lifts and reductions. It can be used if the patient's skin is very elastic, or if it is a smaller reduction. The scar is shorter because we are not removing as much skin. If there are low nipples, deflated breasts, and low skin elasticity, we still do a 't. The important thing to note is that we must look at the surgery as a 'breast gland operation and not just a 'skin operation (which was the old school thought). We have to reshape the breast mound first, and then the skin comes after. This way, patients get longer-lasting results with a better shape and no bottoming out. Patients actually get a lift out of a reduction.

Who is a good candidate for breast reduction surgery?

Anyone who has cosmetic and functional symptoms associated with larger, pendulous breasts: They can't fit into clothing; they have shoulder, back and neck pain; or if they have inflammation in the mammary area from moisture. If enough weight is removed from the breast, than insurance often times will cover those procedures. Smokers are not good candidates.

What can you expect after undergoing breast augmentation surgery?

Most people recovery very quickly. I encourage light activity right away the next day. Strenuous exercise or heavy lifting has to wait until three to four weeks, but patients can go back to work in a day or two as long as their job doesn't require heavy lifting. Choice of bra is important. My patient's wear a special strap for the first few weeks, and they can't wear and underwire bra for a month.

Who is a good candidate for a mini tummy tuck?

Women who have no looseness or laxity of skin above the navel or are not bothered by it. Generally, they have no separation or diastasis of the muscles above the navel. If they are poochy above the navel, then they need a full tummy tuck (full abdominoplasty).

What is Body Contouring?

When is a thigh lift needed?

There are two diff kinds: An inner and outer thigh lift. Nowadays with the obesity epidemic, many people are having gastric bypass and lap band surgeries. Because of the massive weight loss, skin loses elasticity and people are left with poor, hanging skin. In this case, we do lateral thigh lifts usually in combination with a tummy tuck all the way around to the buttock, which lifts up the outer thigh. This is usually done in stages. An inner thigh incision is made at the crotch, along the groin crease, or sometimes, although rarely, the incision can be made vertically, where the pants seam would be.

BREAST IMPLANTS

How do you fix droopy breasts?

It depends on how droopy they are. If the patient has lost volume in the upper part of the breast, but the nipples are still high, then all we have to do is put a small breast implant in. As the nipple starts to drop relative to the breast mound, or below or at the inframammary fold, then we do a breast lift. Or, sometimes we do a combination breast lift with augmentation, depending on the need of volume.

What type of breast implants give you the most natural look?

It's not the type of implant that matters. It depends on the patient, and is a decision made by the surgeon and the patient. Implants have to be in proportion to one's own dimensions for them to look natural. There's really not a big difference in the way saline and silicone looks. However, patients with thin skin tissue and not a lot of muscle to cover may do better with silicone implants because saline has more of a tendency to show wrinkling and rippling. There are also different shapes of both saline and silicone implant. They can be smooth or textured, or have a moderate profile (wider with less projection), or a high profile (which are narrower for the same amount of volume with more projection). Implants have to be tailored to the right proportions or for a certain kind of look.

EYELIFT

How do I fix bags or puffiness under the eyes?

Usually, under-eye bags are due to normal fat that exists because of heredity or age. The membrane weakens and the fat then protrudes or bulges, and the tear trough casts a shadow underneath the bag. Most of the time, under-eye circles are not caused by pigment, but by this shadowing. I don't believe in removing the fat from inside the lid. I'd rather preserve the fat and move it down, filling in the tear trough. This is called a Fat Transposition Lower Eyelid ift. This softens the part between the upper part of the cheek to the eyelid.

How do you correct droopy upper eyelids?

What if you have darker skin, are there any laser resurfacing options?

I use the Thulium 1927, and it seems to work for melasma, but it's not FDA approved for this (it's approved for hyperpigmentation). However, there are new technologies every day with effective predictable results.

Who is the best candidate for facial laser resurfacing?

It depends on your skin type and quality. Fairer-skinned people do better than people with darker or olive skin, they are prone to have pigmentation problems afterwards. You have to know what type of laser to use and what the indications are as to why you are using the laser.

How do you correct droopy upper eyelids?

Can the patient's own fat be used to restore facial volume?

It's commonly done. It works very well and the good part is that it's the patient's own tissue. Usually it's something that is done in stages because some of the fat resorbs; and we have to harvest it from somewhere. It's more for hallows and folds, not for wrinkles. Studies have been done with fat grafting. Since they contain a high proportion of stem cells, the quality of the skin actually looks better when using fat as filler because the stems cells cause regeneration of tissue.

CHIN AUGMENTATION

How can you fix a weak chin?

The simplest way is with a chin implant where a small incision is made under the chin or inside the mouth. I've had the most predictable results with a small, solid silicone implant. There are procedures where bone is cut and slid forward. Some patients even benefit from BOTOX. If they are subconsciously contracting muscles in their chin, then BOTOX relaxes those muscles and the chin comes forward. It's a soft tissue and muscle issue, not a bony one, and the results are temporary.

It is a non-scientific term usually consisting of a breast augmentation or lift combined with a mini or full tummy tuck.

CELLULITE TREATMENT

What is the best cellulite treatment? Do they work?

There are a few kinds of non-surgical treatments such as VelaShape (which uses a combination of Bi-polar Radiofrequency (RF), Infrared Light Energy, plus Vacuum and Mechanical Massage to deploy targeted heat to adipose tissue) and Endermologie (a form of therapy and suction utilizes small rollers which gently massage the area where cellulite has developed). They work, but you have to keep going because the cellulite recurs.

It's generally reserved for those who have loose skin in the upper arm or triceps area, if someone is troubled by skin hanging from the elbow to the armpit when they raise their arm.
